I give permission for the Arkansas H.A.W.K.S. Football volunteers to coach my child while my child performs a number of tasks. I am aware that my child will be involved in a physical activity and will not hold the representative of Arkansas H.A.W.K.S. Football Organization accountable for any injury that my child may encounter.
If my child does encounter any injury, I give permission to the Arkansas H.A.W.K.S. Football Staff to notify medical authorities and get my child medical help. I also know that the volunteer staff will always act in a professional manner.
I am also aware that videos and photos may be taken of my child during the football season. I give permission for these pictures or videos to be used in Arkansas H.A.W.K.S. advertisement to represent youth football.
